Doc Wendler is actually fitting Furillo for the plastic insert worn underneath the cap in lieu of a full batting helmet. You've never seen Carl batting with a full helmet on his head--always the cap with that "thing" underneath.

As kids we put cardboard in our hats so we would be like the players with the plastic in their caps when we played stickball or hardball away from the little league where we had to wear helmets.
